What is Breast Augmentation?

Breast augmentation surgery is an aesthetic surgical operation performed to increase the volume of the breasts. With this operation, breast prostheses are placed under the breast tissue or chest muscles. With these implants placed, the size of the breast tissue is increased and brought to the desired size. With the operation, women get the breast and body appearance they dream.

Patients with smaller breast sizes than desired apply for breast augmentation surgery. It is a frequently used and easily performed surgery today. With breast augmentation surgery, the appearance, size, proportion, and volume of the breasts are corrected.

Breast augmentation surgery is performed in women with structurally small breasts, the absence of congenital breast tissue, and asymmetry between the breasts.

The most valid breast augmentation method used today is the silicone prosthesis method. It can also be done by applying the patient’s fat tissue. However, the success of this method is not as high as the silicon method. The outer surface of the prostheses used in breast prostheses can be made of silicone, and the inner surfaces can be made of different materials.

The size of the bra used by women is determined by the cup size, which varies from A to E, and the circumference of the rib cage, which varies between 70 and 110. The B cup determines the ideal breast volume and the C cup determines the upper limit. A cup and below are candidates for breast augmentation, and D cup and above are candidates for breast reduction.

In which cases is Breast Augmentation surgery performed?

Breast augmentation surgery can be applied to patients who are over the age of 18 (because breast tissue development continues), have good general health, and have complaints or dissatisfaction as follows;

  • Those who think that their breasts are too small for their body,
  • Those who are congenitally or subsequently have unilateral breast development disorder/asymmetry,
  • Those who think about rebuilding the breast tissue lost after breast cancer surgery,
  • Those who want to enlarge their sagging or shrinking breasts after breastfeeding, pregnancy, or serious weight loss,
  • Those who do not develop breast structure due to chest anomaly,

How is the breast size decided?

Breast size is decided between the patient and the doctor. Issues to consider when deciding on breast size;

  • Characteristics of the chest wall,
  • Dimensions of the base of the breast,
  • Position of the breast on the chest wall,
  • The thickness of the breast tissue,
  • Asymmetric breast structure,

factors like these are revealed by making measurements.

The breasts are augmented with a silicone gel called a breast prosthesis and placed behind the breast tissue. Generally, patients cannot describe the breast size they want to achieve and often say the bra size they want (like 85, or 90). However, the sizes of different bra brands can be different. After the operation, the size expressed by the patient as 85 – 90 will not change. Because this measurement refers to the diameter of the rib cage. In turn, the breast sizes expressed as cup size and denoted by the letters A, B, C, D, and E will change. Here, the letter A indicates the smallest breast size, and the letter E indicates the largest breast size. Therefore, the expectation of the patient should be determined very well.

Some patients want a change that their spouses or partners will notice. Others want to be noticed in their social environment. The extent to which the patient will want a change and the level of awareness of this change must be determined. The important thing is to consider a change within the limits of what is aesthetically possible.

Who is NOT A Good Candidate for Breast Augmentation

Breast augmentation surgery should not be applied to patients with the following conditions;

  • Those with a pacemaker,
  • Patients with allergies,
  • Those who gain and lose weight frequently,
  • People with diabetes,
  • Those who are undecided for surgery,
  • Those who smoke or drink alcohol,
  • Those who use blood thinners,
  • Those on ongoing cancer treatment,
  • Patients who are pregnant or at risk of pregnancy,
  • Patients who have problems during the surgical recovery process,
  • People with cicatrization (Wound healing) or keloid problem.
  • Those under the age of 18 (as breast tissue development continues),
  • Those who are considering pregnancy shortly after breast augmentation surgery,
  • Patients with collagen tissue disease or chronic rheumatic diseases,
  • Those who are breastfeeding (at least six months after the end of the breastfeeding process).

What should be considered before breast augmentation surgery?

Before deciding on breast augmentation surgery, the following conditions should be considered;

Breast implants do not prevent breast sagging. Breast augmentation should be applied together with a breast lift.

Conditions such as leakage or tearing may occur after the implant is inserted.

Weight gain and loss after surgery may cause a change in the appearance of the breasts.

Interpretation of mammogram results can be difficult. In the presence of breast implants, special examinations may be required in addition to mammograms.

Health insurance does not cover breast implants, except for the medically necessary and  removal of the breast after cancer. For this reason, breast augmentation operations are not covered by insurance, except for medical conditions. Future tests and imaging fees will be entirely yours, along with the surgeries to be performed. You must be prepared for these.

In some cases, a drain can be placed in the breast tissue. Drains can usually be easily removed the next day.

It is mostly performed under general anesthesia. But, in some cases, it can be done under local anesthesia with light sedation.

Your physical condition should be good.

If you have ongoing treatments, they should be reported to the surgeon and anesthesiologist.

The operation takes approximately 1-2 hours.

Mammography/breast ultrasonography should be done. If the patient is under the age of 40, breast ultrasonography is requested, and if the patient is over 40, mammography is requested in addition to breast ultrasonography.

It is desirable to redo mammography/breast ultrasonography in the first year after surgery.

What are the risks of Breast Augmentation surgery?

Breast augmentation surgery performed by experienced plastic surgeons is among the surgeries that carry the least risk, have the most comfortable, and whose results make physicians and patients smile.

Every surgery has complications and risks. Breast augmentation surgery also has risks. These risks are;

  • Swelling and pain in the breast area,
  • Infection and bleeding
  • Tenderness in the nipples,
  • numbness around the incision,

The risks associated with the prosthesis are as follows;

  • The scar tissue formed around the prosthesis compresses the prosthesis and gives a feeling of hardness to the breast (Capsular contracture).
  • The prosthetic contents may leak out of the balloon. While gel-filled prostheses cause mass formation, saline-filled ones are absorbed by the infiltration tissues and do not cause any harm to the body.
  • The prosthesis can be torn. It can usually occur in traffic accidents, falling from a height, and penetrating tool injuries. It is rarely encountered.
  • Cancer may occur in the breast with silicone prosthesis. The risk of cancer formation in the breast with silicone prosthesis has the same ratio in normal breast.

As a result of prostheses that do not carry CE or FDA approval by incompetent people, the surgery fails and involves numerous risks.

How to prepare for Breast Augmentation surgery?

It is important to consider the following points before the operation:

In the general examination, your physician should be informed about any of your diseases, allergies, medications you use, and surgeries you have had.

Some tests should be done for general anesthesia.

Aspirin-type blood thinners, flu medications, and pain relievers containing Ibuprofen should not be used one week before and two weeks after surgery.

Birth control pills should be completely discontinued one month before and one month after surgery.

Roacuttane etc. drugs should not be used for six months before and six months after surgery.

Since substances such as vitamin E, cherries, garlic, green tea, sour cherries, herbal teas, and flax seeds also cause blood thinning, they should be stopped one week before the surgery and should not be used for at least two weeks afterward.

Alcohol should not be consumed for 48 hours before the operation and for the first week after it.

All diseases you have had in the three weeks before the operation and the drugs you use should be reported to the surgeon and the anesthesiologist.

Smoking should be stopped four weeks before the operation.

Take a bath one night before the operation.

Pajamas with buttons should be brought to the hospital.

Vitamin C should be used for two weeks before and after surgery (for fastly wound healing).

Before the operation, the hair around your nipple should be cut with scissors (pulling them with tweezers may cause folliculitis, it is inconvenient).

Before general anesthesia, eight hours of fasting and 4 hours of dehydration should be avoided.

If an upper respiratory tract infection occurs, the surgery should be postponed.

The operation should not coincide with the menstrual period.

What are the types of Breast Augmentation aesthetics?

Expectations before breast augmentation, desired dimensions, breast structure, and skin consistency are evaluated together and it is decided what kind of application will be made. The methods applied in breast aesthetics are as follows;

Breast augmentation with medical silicone implant:

It is the most commonly used method today. With a detailed examination and three-dimensional simulation, the most suitable silicone size for the patient is determined.

Breast augmentation with ready-made fillers:

In this method, the filling is perceived as a foreign body in body. The resulting infections can cause irreversible damage. In such an event, it is almost impossible to clean the filling without damaging the nozzle.

Breast augmentation with fat fillers:

It is an application made with fat injection. The breast consists of breast tissue and adipose tissue. Fat tissue, which is less in youth, increases as age progresses, causing sagging in the breast. Breast augmentation with adipose tissue, on the other hand, is to cause a sagging breast over time instead of a live breast.

Hybrid or composite breast augmentation:

It is the process of partial fat filling around the silicone implant placed on the muscle in thin-skinned patients to camouflage it. This method is preferred in patients whose breasts have been removed due to cancer, whose skin is thin and whose muscles are inadequate.

For the reasons mentioned above, the most preferred and safe breast augmentation method today is breast augmentation with medical silicone implants.

What are the features of the aesthetic breast?

Aesthetic breasts should have the following features;

  • The breasts should be located symmetrically to each other on the chest when viewed from the front.
  • The teats should be equidistant from the midline and the teat diameter should be approximately 38-42 mm.
  • When viewed from the side, the area from the armpit crease to the nipple covers 45% of the breast; The area from the nipple to the inframammary fold should constitute 55% of the breast.
  • There is no dimple or protrusion in the upper part of the breast in the natural breast. This area should be flat.
  • The concept of decollete expresses the fullness that occurs with the outfit. There is no decollete appearance when naked on the natural breast.
  • The breasts should be upright so that the pen does not stop there when the pen is placed on the folded area under the breast.
  • Breast size depends on the person’s height, weight, and body structure. However, B or C cup sizes are generally more suitable for women in our society.

What is the cup size in the breast?

The size of the breast is expressed in the “cup” measurement unit. This size is expressed by the letters on the bra. For example, a bra marked 80B indicates that the chest circumference is 80 cm and the breast size is B Cup. The number here indicates the circumference of the rib cage, not the breast size.

European, American, and Asian cup sizes differ slightly among themselves. The breast size of an average woman in Turkey is expressed as B Cup and C Cup.

How to choose the implant (silicone) size?

With the pre-operative examination, the diameter, shape, and size of your breasts are examined, and the most suitable method is explained. You choose the breast implant size together with your surgeon.

It would be useful to research the subject before coming to the examination. As you know, bra sizes consist of a number and a letter. The number indicates your under-breast rib cage circumference, and the letter indicates your breast size. You can get an idea of the size by trying different sizes of bras. The thing you should pay attention to is that the measurements may differ from country to country. A prosthesis that may be inadequate for you may be large for someone else. There are thirteen different types of a prosthesis in the same volume. Therefore, the volumes of prostheses for other people should not be a reference for you.

While your body (the width of the rib cage and your height are decisive) decides the width and length of the implant, the height is determined by your taste and the experience of your surgeon. Is there any pain after breast augmentation surgery?

Breast augmentation prostheses are placed under or over the muscle. As a result of prostheses placed on the muscle (subfascial), there is almost no pain. Pain may occur as a result of implants placed under the muscle. In this case, the situation is brought under control with the painkillers recommended by your surgeon, and there is no unbearable pain.
