What is Rhinoplasty?

Rhinoplasty, also known as rhinoplasty, can be performed especially to reduce the size of the nose and to correct its root or edge. This procedure is usually performed under anesthesia. The operation is done by cutting or adding nasal bones or cartilage.

Another procedure for narrowing the nostril, narrower and more prominent nasal bridge, or nasal tip improvement is known as alarplasty. This procedure is performed to reduce the width of the nostril or make the nostril narrower, providing a narrow and prominent nasal bridge and nasal tip aesthetics.

With rhinoplasty, nose size, nose angle, nasal tip, and nasal bridge problems that cause discomfort in your nose, the discomfort that prevents comfortable breathing and nasal problems that the patient is not satisfied with can be eliminated. In which cases is rhinoplasty performed?

Rhinoplasty is usually performed for aesthetic purposes. In addition, it can also be performed to correct congenital or traumatic abnormalities in the nasal structure. Rhinoplasty can be performed in patients with the following conditions;

  • For aesthetic purposes,
  • To lift the tip of the nose,
  • For nostril reduction – enlargement,
  • For congenital and acquired deformations,
  • For people with respiratory problems,
  • For those with a sunken noses,
  • For those who want to gain nasal symmetry,

Who cannot undergo rhinoplasty?

For operations to be applied to nasal problems caused by congenital or traumatic reasons, the patient must be over 18 years old. Our patients should not have health problems in organs such as the heart and lungs that will prevent surgery. Rhinoplasty is not applied to those with serious nasal infections and tumors in the nose. In addition, pregnant and breastfeeding women should consider plastic surgery after these periods are over. Patients who have had rhinoplasty surgery should have at least 1 year after the first operation to have a second operation.

Other conditions in which rhinoplasty cannot be performed are as follows:

  1. People with body dysmorphic disorder;

It is a psychiatric disorder. Such patients are not satisfied with their bodies in any way. No change will please them, the very small defects they see will be very big. These patients should undergo psychiatric treatment before rhinoplasty operations.

  1. Those with obstructive sleep apnea;

Obstructive sleep apnea is frequently recurring breathlessness during sleep. These patients may encounter unwanted problems after the operation. First of all, these treatments should be done.

  1. Those with bleeding disorders;

Nasal bleeding is one of the common complications after rhinoplasty. In the face of such a complication, the plastic surgeon intervenes in the nasal vein.

Patients with blood clotting problems should inform the plastic surgeon before the operation. Patients who experience blood clotting problems due to the medications they use should stop these medications for a while before and after the operation.

What are the complications of rhinoplasty?

Like any surgical procedure, rhinoplasty can involve some complications. These include:

Pain: Pain after rhinoplasty is normal and can usually be controlled with painkillers.

Bleeding: Bleeding after rhinoplasty is considered normal. Very serious bleeding does not occur, and bleeding is stopped with tampons.

Nasal congestion: After rhinoplasty, nasal congestion may occur as a result of temporary adhesions in the airways. It can be solved with procedures to open the airways.

Very rarely, perforation of the septum nasalis (the structure that separates the nostrils) can be seen. This causes noise when breathing. Treatment is difficult and may require surgery.

Changes in the skin: Skin changes after rhinoplasty are rare and usually temporary.

Serious complications after surgery are very rare. For this reason, it is important to consult a specialized plastic surgeon in advance.

Problems such as deformity and not liking the shape of the nose may occur after the operation. This is the most common complication.

What is the anatomy of the nostrils?

The nostrils are where breathing begins. They connect the nasal cavity and the posterior mouth, allowing air to flow between the inside of the nose and the posterior mouth. Inside the nose, in front of the nostrils, is the nasal mucosa. The mucosa contains tiny glands to moisturize and filter the inside of the nose. Behind the nostrils are the bones and cartilages of the nose. In this area, there are muscles to change the size and shape of the nostrils.

The anatomy of the nostrils plays an important role in breathing, sound production and balancing intranasal pressure, as well as in nasal surgery.

Two “n” shaped cartilages make up the tip of the nose, the wings of the nose and the nasal base. These cartilages are called “external valves”.

The middle part is called “Columella” (outside the nose, where the nostrils meet). Just behind the columella is a thick cartilage called the “septum” that separates the two nostrils. On the part of these cartilages extending to the back of the nose, there are triangular, leaf-shaped cartilages called “internal valves” that form the roof of both nostrils. Continuing these cartilages are the nasal bones.

What are the types of rhinoplasty?

There are two types of rhinoplasty performed under general anesthesia;

Open operation technique:

This method is performed by lifting the skin of the nose and revealing the bone and cartilage structures that form the tip and back of the nose. This technique may be suitable for major nose changes.

Closed operation technique:

In this method, the area to be intervened is accessed only through incisions made through the nostrils. This technique may be suitable for minor nasal changes.

What are the surgical methods in rhinoplasty?

Rhinoplasty, commonly known as rhinoplasty, is a surgical technique used to change the shape and size of the nose. The most common surgical techniques used for rhinoplasty are as follows:

Reduction Rhinoplasty

The dorsal part of the nose has a bone structure above and a cartilage structure below. In this technique, the protrusions on the back of the nose (nasal arch, hump) are cut and removed to give the nose a natural and aesthetic appearance. During this procedure, the weakness in the bone-cartilage transition area must be repaired. If there is not enough repair here, it may cause aesthetic and functional problems during healing.

Preservation Rhinoplasty

In this method, the protrusions on the back of the nose (nasal arch, hump) are corrected by displacing them into the nose without cutting and removing them as in the reduction rhinoplasty method. In this procedure, the integrity of the protrusions is preserved.

In the protective rhinoplasty method, two applications, push down and let down, can be used together or separately. Push down is preferred for mild changes and let down is preferred for severe changes. In this method, the natural structure of the nasal dorsum is preserved.

This method; It is applied in patients with no obvious bone protrusion or asymmetry, predominantly cartilaginous arches, no obliquity extending to the back of the nose and no previous surgery.

Ultrasonic, Piezo, Diamond and Precision Rhinoplasty

These are the methods used to shape only the bone structures of the nose. With this method, cutting and shaping is performed with micro vibrations on the bone structure and the surrounding soft tissues are not damaged. The biggest advantage is that the bones cut with this method heal in a faster time.

Tiprinoplasty (tipplasty)

It is a method in which only the deformities at the tip of the cartilage structure are corrected without any intervention to the nasal structures.

It is applied to correct nasal tip drooping and asymmetries, to reduce the nasal tips and nostrils, to correct the cartilages that narrow the nasal entrance, and to clarify the nasal tip point.

Since there is no intervention to the bone structure during the operation, bruising and swelling around the eyes are usually not seen. Since a short-term bandage is applied, the patient’s return to daily life is fast.

The most appropriate surgical technique is determined by a specialist plastic surgeon after an examination.

What are the non-surgical methods in rhinoplasty?

The effects of non-surgical rhinoplasty methods can be temporary and last from a few months to several years. A surgical rhinoplasty provides more permanent results but requires higher costs and a longer recovery period.

Nose Filling

It is an application used to correct mild nasal defects. In this method, nasal deformities, asymmetry, and depressions are corrected.

Gel-like, injectable hyaluronic acid is used. This filling maintains its effect between 4 months and 3 years.

Rope Hanging Method (French Strap)

This method is applied under local anesthesia. The low nasal tip can be eliminated with the French hanger method. A small incision is made in the nose and the tip of the nose is suspended to the cartilage tissue. There may be mild pain after this procedure, but there is no bruising. It is applied in 15 minutes in a clinical environment. The patient can return home immediately after the application.

Rhinoplasty with filler or botox:

This technique involves updating the inner face of the nose without going through the nostrils. It is suitable for minor nose changes.

Hydrogen peroxide peeling:

It is a method that removes the top layer of the skin. Hydrogen peroxide peels the skin and a new and healthy skin tissue emerges from underneath. The bad appearance of the skin is removed and a better appearance is obtained.

Radiofrequency:

In this method, radio waves are introduced into the tissue. Thanks to these radio waves, shrinkage and hardening of the tissue occur. Thus, it causes shrinkage in the tissue to which it is applied.

Laser treatment:

In this method, the thick nasal skin can be made thin with a “CO2 laser”. With this application, the superficial thick layer of the skin is peeled off. With this technique applied to patients with thick skin, a healthier skin layer emerges from underneath.

How is the most suitable nose shape for your face type determined?

The most suitable nose shape for your face type is determined by a plastic surgery specialist. The specialist will examine the general symmetry of your face, nose structure, and other facial features. Your wishes and expectations will also be taken into consideration. After rhinoplasty, the patient’s nose, forehead, lips, cheekbones, lower jaw, and jowls should be in harmony.

What should be done before rhinoplasty surgery?

The following should be done before rhinoplasty surgery;

Attention should be paid to the choice of aesthetic surgeon and clinic;

Rhinoplasty surgery is an aesthetically risky surgery. Information about the doctor’s experience, training, and experience should be obtained. In addition, the safety, hygiene, and service quality of the clinic should also be taken into consideration. You should also communicate with the doctor and share your questions and concerns. Otherwise, if you do not choose an experienced plastic surgeon, you may need a correction operation. 

Preliminary examination and analysis should be done for the surgery;

First of all, patients’ expectations about rhinoplasty should be determined. If any, medications and chronic diseases should be learned. It should be determined whether there are breathing difficulties, nasal and nasal discharge, and headache that may adversely affect the operation. The nasal structure should be physically examined. Some blood tests and imaging should be performed.

Three-dimensional imaging should be done by taking photos of the nose;

Photographs of the patient are taken from different directions to show the structure of the nose and transferred to the computer environment. By adding the patient’s expectations, the shape that the nose will take at the end of the operation is determined with the three-dimensional imaging system. Finally, these images are shared with the patient and the operation is decided.

The use of some medications should be suspended;

The use of medications that may pose a risk for the operation (especially blood thinners, and painkillers such as aspirin) should be discontinued one to ten days before the operation.

Blood thinners should not be used for a period to be determined by the plastic surgeon after the operation.

Food consumption should be stopped a while before the operation;

Since the operation will be performed under general anesthesia, food consumption should be stopped 8 hours before the operation.

Food and drinks that are likely to cause gas and edema should not be consumed;

Consumption of gas-forming foods such as milk, legumes, mixed herbal teas, broccoli, and legumes should be stopped a few days before the operation. Care should be taken to consume foods such as mineral water, pickles, olives, and cheese, which have high sodium content and can cause edema.

Smoking and alcohol consumption should be stopped;

Both smoking and alcohol consumption have many harms proven by scientific data. Nicotine in cigarettes delays wound healing as it reduces the amount of oxygen in the blood. Both smoking and alcohol suppress the immune system and cause infection. Stopping smoking and alcohol consumption for one month before and after the operation will increase the success of the operation.

It should be shared with the patient that it is necessary to be patient for the final image of the nose;

After the operation, the patient achieves the desired nose appearance after 6 months to 1 year. There should be no hurry in this regard and patiently wait for this period to expire.

Women should tell their menstrual period the physician;

Women are more sensitive to stress and pain during menstruation. The body during menstruation

How is rhinoplasty surgery performed?

Various applications are performed for nose aesthetics. Some of these are performed with an operation, while others are performed without an operation. The rhinoplasty procedure applied with the operation method is performed in 2 ways, open and closed.

Preparation for surgery

  • Follow the rules before the operation.
  • Be at the hospital at the agreed time.
  • Necessary tests (blood, biochemistry) are done.
  • Pulse, blood pressure, and body temperature are measured.

If the measurement and test results are appropriate, preparation begins.

The nose is photographed from the front, top, bottom, and sides and simulated with 3D imaging. An image close to the image expected to be formed at the end of the operation is obtained. While this image helps the patient’s expectation, it also guides the physician for the surgery. 

  • General anesthesia is applied to the patient.
  • The surgery is performed.

There is no routine application of nose surgeries. It is planned individually according to the patient’s nose shape, characteristics, and preferences. It is performed with two methods, open and closed.

Open rhinoplasty operation: 

It is performed by making a small V-shaped incision in the lower part between the nostrils (lower part of the columella). This incision provides the surgeon with a wider angle of vision.

The most preferred method is an open rhinoplasty. It is applied in cases with advanced structural disorders or in cases where rhinoplasty surgery has been performed before but failed. A V-shaped incision is made in the lower part of the columella. The skin of the nose is lifted. In this application, the bones, cartilage, and muscles of the nose are seen. Necessary procedures are performed through this incision. After the procedure is completed, the incision is closed with sutures. However, the operation time is longer than closed rhinoplasty. Edema and bruises in the nose also disappear for a long time. As a result of the incision, a scar remains, although it is not very obvious.

Closed rhinoplasty operation:

It is a preferred method for simple surgeries. A 3 – 4 cm incision is made on the inside of the nose. The procedures to be performed are performed without lifting the nose. It takes less time than open rhinoplasty and recovery is faster. Since structures such as cartilage and bone are not directly visible, it is difficult to perform the procedure compared to open rhinoplasty. While it is easy to intervene in the bleeding vessel in open rhinoplasty, this process is difficult in the closed technique. There are no stitch marks in the operation performed with this technique.

General procedures performed during surgery

The procedures performed in both operations are similar.

  • If there is a curvature in the cartilage (septum) between the nasal cavities, it is corrected.
  • The condition that causes blockage in the nose is eliminated.
  • The nasal arch is eliminated by filing.
  • Necessary procedures are performed in the nose and the incision is sutured.
  • A tampon may not be placed in the nose after the operation. Silicone tampons are preferred if they are to be placed.
  • The back of the nose is plastered,
  • The operation is terminated by placing a cast or splint.   

The duration of the operation may vary between 1.5 hours and 4 hours depending on the degree of nasal defect, the technique applied, and whether there is an extra procedure. The patient stays in the hospital overnight and is discharged the next day.

Nostril reduction surgery

It can be performed alone or during rhinoplasty surgery. It is a surgery that can also be performed alone with local anesthesia.

Non-surgical nostril reduction

Non-surgical nostril reduction is not a very effective procedure. Botox application to the muscles that pull the wings of the nose and filling on the sides of the wings of the nose can be partially beneficial.

How is nostril asymmetry corrected?

If one of the nostrils is higher or more external, a unilateral or bilateral intervention can be planned, taking into account the relationship of the wings with the tip of the nose and the nose. It can be performed with local anesthesia alone.

Is nostril enlargement possible?

Small nostrils are not a common condition. It can usually occur congenitally and sometimes after previous nose surgeries. If it is due to previous surgical procedures, it should be planned under general anesthesia. If there is congenital stenosis, the operation is performed under local anesthesia. After the operation, nostril expanders should be used and re-narrowing should be tried to be prevented.

Is it possible to thin the wings of the nose?

The thickest part of the nasal skin is the wings of the nose. It thickens as it descends from the root of the nose to the tip of the nose and becomes thickest on the wings. Thinning of thick nasal wings is performed by a surgical procedure to be applied around the nostrils.

What should be considered after rhinoplasty surgery?

The biggest factor in the result after the surgery is the experience of the physician. In addition, factors such as the general health status of the patient, the presence or absence of chronic diseases, the level of changes in the nose, the characteristics of the bone and cartilage tissue in the nose, smoking, and alcohol addiction, and nutrition are also effective.

 The following should be considered after an aesthetic nose job;

The control examination called by the physician should be attended;

Physician controls should not be interrupted for follow-up of bleeding and bruising, dressing, or removal of stitches, splint, tampon, and adhesive tape.

It should not be forgotten that bruises and edema that occur after surgery are temporary;

Bruising on the nose and under the eyes are expected results. Bruises disappear after 10 days, and if edema has occurred, it disappears after three weeks. Swelling of the eyelids after the operation should also be considered normal.

Drugs recommended by the physician should be used for pain and edema;

Medications recommended by your plastic surgeon should be used for pain and edema that will occur after the operation.

Edema and pain in the nose should be reduced by applying cold treatment;

Cold or ice application on the cheek and around the eyes will reduce pain while preventing swelling and bruising. You can do this application for three days.

The nose should be protected from sunlight;

The nose should be protected from sunlight after the operation to prevent permanent color changes in the nasal skin. Edema reduction may also be delayed due to the sun.

Sunscreen (at least 30 factors) can be used to protect from the negative effects of the sun.

Very hot foods should not be consumed;

Consuming very hot food is not desirable after the operation. Hot foods may prevent the reduction of edema. For this reason, food should not be too hot in the first weeks.

The nose should be massaged as recommended by the physician;

For the edema in the nose to disappear quickly, nasal massage should be performed 1 month after the operation as described by the plastic surgeon. This message should be repeated 3-4 times a day for 3-6 months.

Gauze should be placed between the upper lip and nose;

Bleeding within a few days after the operation is normal. For this reason, blood-absorbing gauze should be placed between the upper lip and nose.

It should wait for 1 year for the final appearance of the nose;

The desired nose appearance is achieved between 6 months and 1 year after the operation. So don’t worry about “why I couldn’t get a nose like I wanted” shortly after the operation.

Employees should take 1 week off;

You can stand up immediately after the operation and do daily work within 1-2 days. However, we should rest for a week and then return to our routine life. This one-week leave will help protect our noses from impacts.

Smoking and alcohol should not be used after surgery;

Since smoking and alcohol negatively affect the immune system, the body becomes vulnerable to infection. Smoking has negative effects on wound healing. Both smoking and alcohol delay wound healing and may open the door to infection.

Care should be taken with the lying position, do not bend too much

When lying down, the head should be above the level of the heart and the patient should lie on the back. 2-3 pillows should be placed under the head. Right and left turns should be in a way that does not affect the operation area. Acting in this way will reduce pain, bleeding, and edema formation.

Bending over will increase blood pressure in the head area and cause edema and pain. Therefore, we should not tie shoes while bending over.

Avoid heavy work and sports for a few weeks;

Since heavy work will weaken our bodies, there is a possibility of damage to the operation area. We should avoid it.

The doctor’s advice about showering should be followed;

The splint and tampon attached to the nose should not get wet. In the first days, the head area should be bathed without washing, and after the splint is removed, it should be bathed normally.  The splint is removed within 2-9 days after the operation.

Do not spend time in hot environments;

Heat prevents the reduction of edema and increases bleeding. For this reason, you should stay away from hot environments (such as Turkish baths, saunas, solariums, and hot springs) for 4-6 weeks.

Avoid movements and traumas that put pressure on the operation area;

Behaviors such as blowing your nose, straining excessively in constipation, brushing your teeth hard, and laughing excessively can damage the operation area. Glasses should not be worn for a while;

If there is no urgent need, glasses should not be worn for 1 month after the operation. 

  • A diet that will not adversely affect the operation area should be chosen;
  • Soft and liquid foods should be preferred for a few days after the operation. Foods with high salt content should also be avoided as they may cause edema.
  • Do not stand up for 4-6 hours after the operation.

  • Recovery starts in the first week.

An arched nose is defined by the formation of an arch in the middle part of the nasal cartilage or bone, or in some cases both. It is a disorder that is not understood when looking at people from the front but is visible when viewed from the profile. It is usually congenital and associated with genetic factors. It distorts the appearance of the nose and may cause the tip of the nose to appear low. It is considered both an aesthetic problem and an inability to breathe and can be corrected with rhinoplasty. How is arched nose aesthetics performed?

During this period, it is recommended to lie down with his/her head elevated. In this way, pain, bleeding, and swelling can be prevented.

In non-surgical rhinoplasty, filling materials or botox are used. Non-surgical application is suitable for mild protrusion, collapse, or curvatures. In such a case, correction can be performed in 30 minutes under local anesthesia.

What is Nasal Tip Aesthetics?

Nasal tip aesthetics (Tip plasty) is an aesthetic operation performed in patients who do not have any deformation in the nasal bone, but who have coarseness, curvature, width, irregularity in the anterior cartilage part of the nose or width in the nostrils, without touching the nasal bone in any way, only to restore the cartilage part in the front of the nose to the desired appearance.
